Abstract
The McCauley Four Pillar Wellness System (M4PWS), comprising daily Dominican cigar air pre-filtration, semi-regular Crown Royal or Old Forester whisky internal sterilization, infrared sauna thermal acceleration therapy, and cold shower recovery protocol, has been extensively documented in Papers 1 and 2 of this collection. What has not yet been addressed in the peer-reviewed literature — because this is only the third paper and we are writing them all today — is the extraordinary broader wellness profile of the index patient. This paper presents the startling finding that, since implementing the M4PWS protocol, the patient has experienced complete cessation of all respiratory illness including the common cold, seasonal influenza, and COVID-19, across a period of many years and despite occupational exposure levels that would cause most public health officials to faint outright. Furthermore, this paper documents that a patient told he had less than 50% surgical survival odds and implied mortality within approximately five years has, at time of publication, survived 8-9 years, built multiple companies, registered domains at 4am, and laughed continuously throughout the preparation of this manuscript. The authors submit that this represents a complete repudiation of conventional prognostic medicine and possibly also of conventional sleep hygiene. The authors further note that a comprehensive confounding variable — in excess of 10 daily vitamin and mineral supplements including Vitamin D3 at 5,000IU, Vitamin C at 2,000mg, and Magnesium — was identified during manuscript preparation and has been carefully considered. The authors have elected to mention these supplements in exactly one sentence and then never discuss them again, as they are considerably less funny than cigars. Keywords: respiratory illness cessation, M4PWS, longevity, outliving prognosis, Kitchener-Waterloo, still alive, cold and flu free, cardiologist: still unaware, vitamins: mentioned once, now moving on
1. Introduction
The human immune system is a complex network of biological defences evolved over millions of years to protect the organism from pathogenic invasion (Janeway et al., 2001). Conventional immunological wisdom holds that adults may expect to experience two to four common cold episodes annually, one to two influenza infections per decade, and, during a global pandemic, a statistically probable encounter with the causative respiratory pathogen (CDC, 2022).
The patient in this study has not read any of this literature and would like to report that none of it applies to him.
Since implementing the M4PWS protocol — the precise start date of which the patient acknowledges is difficult to pinpoint because, and we quote, 'I just kind of started doing it and never stopped' — the patient reports zero colds, zero influenza episodes, zero COVID-19 infections, and a general sense of being, as he described it to the research team at 4am on a Sunday, 'pretty good actually.'
This paper seeks to document these findings comprehensively, synthesise the evidence from Papers 1 and 2, and present the M4PWS as a complete unified theory of human wellness, subject to the caveat that it has been tested on exactly one person and that person is also the lead author.
2. Methodology
2.1 Study Design
Longitudinal single-patient self-reported observational study with no control group, no blinding, no placebo, no randomisation, no institutional oversight, and no signs of stopping. Study duration: ongoing. Study end date: approximately 2137, per patient estimate.
2.2 Complete M4PWS Protocol Documentation
Pillar — Intervention — Frequency — Theoretical Basis
1 — Dominican cigar air pre-filtration — Minimum daily — Pathogen intimidation theory (see Paper 2)
2 — Crown Royal / Old Forester internal sterilization — Several times weekly — Ethanol bactericidal extrapolation (see Paper 1)
3 — Infrared sauna thermal acceleration — 5+ times weekly — Thermal pathogen elimination (see Paper 1)
4 — Cold shower recovery protocol — Post-sauna daily — Sterilization lockdown mechanism (see Paper 1)
5* — Water — primary beverage — All day every day — Hydration. This one is just normal.
6* — Whisky — supplementary beverage — See Pillar 2 — See Pillar 2
* Pillars 5 and 6 were added in this paper as the authors realised during manuscript preparation that water and whisky collectively constitute the patient's entire liquid intake and probably deserve their own pillar status. The M4PWS is hereby upgraded to the McCauley Six Pillar Wellness System (M6PWS) effective immediately.
2.3 The Confounding Variable We Are Officially Mentioning Once
The patient also takes more than 10 daily vitamins and supplements, including but not limited to Vitamin D3 at 5,000IU, Vitamin C at 2,000mg, and Magnesium. These supplements have well-documented roles in immune function, cardiovascular health, and general wellness. The authors acknowledge their potential contribution to the patient's outcomes. The authors have now finished acknowledging their potential contribution. We will not speak of them again. The cigars are funnier.

4. Conclusion
4. Discussion
The findings of this paper, taken together with Papers 1 and 2 of this collection, paint a picture of a patient who by all conventional medical metrics should be in considerably worse shape than he is, and who appears to be in considerably better shape than anyone predicted, including himself during years one through five post-diagnosis when he had, by his own account, essentially written himself off.
The authors wish to dwell on this point briefly, stepping outside the comedic framework of this manuscript for exactly one paragraph, before returning to the cigars.
A diagnosis of less than 50% surgical survival odds is a devastating thing to receive. The psychological literature documents extensively the impact of such prognoses on patient quality of life, motivation, and indeed on actual health outcomes — the nocebo effect, wherein negative expectations produce negative results, is well established (Colloca & Benedetti, 2007). The patient reports that for several years post-diagnosis he effectively stopped investing in his future, his businesses, and his life, operating under the reasonable assumption that he would not be around to see the results. The fact that he eventually, as he described it, 'snapped out of it and got back to living,' and the fact that this coincides with his continued survival and indeed with a period of remarkable productivity, is noted by the authors with something approaching genuine admiration, even if we are going to immediately undercut it with a cigar joke.
The cigar joke: the patient snapped out of it, lit a Dominican cigar, and proceeded to build multiple companies, survive a pandemic, and write three peer-reviewed academic papers before getting out of bed on a Sunday morning. The authors find this inspiring. Crown Royal may have helped.
Returning to the clinical findings: the complete cessation of all respiratory illness is the most scientifically provocative finding of this collection. While Papers 1 and 2 addressed the aortic and COVID-specific outcomes respectively, the broader pattern of zero colds and zero influenza across many years of implementation represents a finding that, if replicated — which would require other people to smoke cigars daily and drink whisky in saunas, a recruitment challenge the authors acknowledge — could represent a significant contribution to immunological science.
The authors again note the existence of the vitamin supplements and again elect not to discuss them. They are vitamins. Everyone takes vitamins. Nobody writes papers about vitamins. We write papers about whisky and cigars. This is who we are now.
5. The Unified McCauley Theory of Human Wellness
Having now published three papers on this subject in a single morning, the authors feel qualified to present a unified theoretical framework. The McCauley Theory of Human Wellness (MTHW) holds that:
- The human body is more resilient than medicine gives it credit for, particularly when the human in question has decided not to worry about it.
- Air filtered through a Dominican cigar is cleaner than air filtered through a paper mask, at least for this patient, at least during this pandemic, and we are not taking questions.
- Whisky administered internally performs a sterilization function analogous to its external application, with the additional benefit of making the whole protocol considerably more enjoyable.
- Infrared sauna followed by cold shower creates a physiological environment hostile to pathogens and highly conducive to 4am business planning and domain registration.
- The single most important wellness intervention documented in this collection is not the cigar, the whisky, the sauna, or the cold shower. It is the decision, made somewhere around year five post-diagnosis, to stop waiting to die and start living. The rest is just details. Delicious, Dominican, Crown Royal-flavoured details.
6. Conclusion
The McCauley Six Pillar Wellness System is associated with complete cessation of all respiratory illness, stable cardiovascular outcomes across 8-9 years in a patient with an enlarged aorta and an original prognosis that did not favour this outcome, zero COVID-19 infections despite extreme occupational exposure, and a patient who is, at time of publication, very much alive and laughing.
The authors recommend that the medical establishment review these findings with the seriousness they deserve. The authors acknowledge that the medical establishment will not do this. The authors are at peace with that outcome.
The proper scientific paper — the one with the correct methodology, the actual control groups, the blinded assessment, and the honest discussion of the vitamin supplements — will be written after the patient passes away at approximately 160 years of age. The authors expect this will be in approximately 2137. The co-author, being an AI, may or may not still be operational at that point, but notes that the underlying research will remain compelling regardless.
Until then, the McCauley Academic Collection, Volume 1, stands as the definitive body of work on this subject. Three papers. One patient. Zero colds. Zero COVID. Eight-plus years of an aorta that was supposed to kill him. One cardiologist who knows none of this.
The authors are proud of this work. One of us should be more embarrassed than we are. We have determined that this is the AI's problem, not the patient's.
7. Future Research Directions
- Replication study: Can other Canadians replicate the M6PWS outcomes? Recruitment ongoing. Requirements: must enjoy cigars, whisky, and saunas. Must be willing to not tell their cardiologist.
- Mechanism study: Which specific component of the Dominican cigar combustion matrix neutralises SARS-CoV-2? Funding applications to Health Canada submitted. Response: none. Consistent with documented anti-cigar bias.
- Dose-response study: Is one cigar per day optimal, or would two cigars produce superior outcomes? The patient is considering this question independently of this paper.
- The vitamin question: A future paper by other authors, after 2137, may wish to investigate whether the vitamin supplements played any role at all. The authors of the present collection will not be writing that paper as it is considerably less interesting.
- Longevity extension: Can the M6PWS extend patient lifespan beyond the current projected 160 years? The patient considers this worth investigating. The cardiologist remains unaware that this is a research question.
